Since 2017, I have been using a Risk Assessment template and Trail Card that were developed in Chinook Council using standards borrowed from the energy sector. They deal with their fair share of risks and the trail card and template make filling out risk assessments very intuitive.
The template has a space for the version date of the BP&P that you used to create the document in case it changes before the event. It also has a sample line already filled in for you.
